If you step out of the water onto a rock a failure happens Geralt is forced back into the water and suddenly can't dive anymore. This I could reproduce serveral times. The only way I can get under is to wait for a monster to attack me which puts me just under the water line so I can start swimming under water. If I go up to surface to get air, same thing, can't go back under unless a monster hits me.
