WebDosage/Direction for Use. Type 2 DM monotherapy & add-on combination therapy 10 mg once daily. Combination w/ insulin or insulin secretagogue Lower dose of insulin or insulin secretagogue may be considered. Initial combination therapy Dapagliflozin 10 mg + metformin 500 mg once daily. Heart failure & CKD 10 mg once daily. WebThere is limited experience with FORXIGA in patients with severe renal impairment (eGFR <30 mL/min/1.73m2) or ESRD. FORXIGA is contraindicated in patients with an eGFR less than 30 mL/min/1.73m2, ESRD or patients on dialysis (see CONTRAINDICATIONS). Special Populations Pregnant Women: FORXIGA must not be used in pregnancy.
